About Connect2Care's Aged Care Support
This client-centered approach ensure that the elderly receive the support they require in order to help maintain their independence & dignity. At Connect2Care we review and update our client care plans on a regular basis to reflect their changing needs and skills. This consistent revision aids in ensuring that clients feel comfortable as they age.
In addition to our direct care, at Connect2Care we actively teach clients and their families how to manage health conditions, use assistive technology, as well as take precautions to try to minimise hazards like falls. The driver of this approach is that it empowers both clients and caretakers to give them improved confidence and control over day-to-day challenges.
