At Connect2Care, we are more than just an NDIS service provider. We are a family-owned and operated business, with decades of experience in the disability sector and a passion for making a positive difference in the lives of people we work with.

Our History
Connect2Care was founded by health professionals with a shared passion for helping people with a disability reach their highest potential.
What started in May 2019, as an NDIS registered provider offering predominantly Physiotherapy and Occupational Therapy services in Melbourne, has grown to become a national, multidisciplinary team of allied health professionals supporting both children and adults with disabilities.
We offer predominantly mobile services throughout Australia, and our therapists have extensive experience working in homes, schools, and other community-based settings. We also provide in-centre therapy at our metropolitan hubs in Melbourne, Brisbane and Sydney, as well as Telehealth appointments.
The driving force behind our success is the combination of exceptional service and building good relationships with our clients and key stakeholders, with a focus on helping our clients to realise their goals and aspirations.

Our practice was specifically established to meet the needs of people living with disabilities in our community. As a registered National Disability Insurance Scheme (NDIS) provider, we offer allied health therapeutic support, including Occupational Therapy, Speech Pathology, Exercise Physiology, Physiotherapy, Psychology, Allied Health Assistants, Social Workers, and Dietetics.
We understand that navigating the NDIS can be a challenging and overwhelming experience. This is why our experienced support team will help assist with the referral process.
