Connect2Care is a family-owned and operated provider of therapeutic services focused on meeting the needs of children and adults living with disability in our community.
Our therapists are highly educated and use evidence-based therapies to help improve the lives of our clients and their families.
We work as a multidisciplinary team to provide the best quality allied health care, including Occupational Therapy, Speech Pathology, Exercise Physiology, Physiotherapy, Psychology, Social Workers, and Dietetics.

Connect2Care comprises a leading team of clinicians, support staff, and other NDIS service providers. We are continually looking to expand our services, with a focus on growing our team of allied health professionals across Australia.
We value teamwork, trust, fostering growth, listening to our client’s needs, and being part of the communities that we live and work in.
Our new graduate program spans 12 months and helps create confident and experienced allied health professionals. We have a strong focus on providing regular one-on-one supervision, mentoring and continued learning opportunities.
Connect2Care is a leader in providing NDIS jobs in Australia. We have a substantial framework to provide quality in-house professional development, as well as support our staff to seek external professional development from leaders in the fields of disability allied health care services.
Staying true to our standards as registered NDIS providers and allied health professionals and conveying them throughout the company is also of high importance. We put a strong emphasis on learning and growth from both a clinical perspective and non-clinical, especially in the area of NDIS therapies and provide ongoing support and mentoring for you to do so.

Connect2Care have multi-disciplinary teams based in Victoria, New South Wales, Queensland, South Australia, the ACT, and Western Australia. We offer a predominately mobile allied health service, allowing therapy to be provided within one’s own environment, whether that be at school, home or a local community setting. This ensures our NDIS service providers can more effectively help build strength and skill where our clients spend most of their time.
In addition, we have state-of-the-art hubs in Melbourne, Brisbane, and Sydney, providing a safe and comfortable place to support clients who have a preference to come into our practices.
What’s better, your search for “NDIS providers near me” does not have to be hundreds of kilometres away. We have an additional model of care offered by our team of registered NDIS providers – the telehealth, where we can support clients who are unable to access our face-to-face services, including NDIS therapies, and provide them the opportunity to engage using teleconferencing tools. This is particularly beneficial for clients residing in rural or remote areas, as well as those individuals who may be immunocompromised.

Our practice was specifically established to meet the needs of people living with disabilities in our community, and we are a proud National Disability Insurance Scheme (NDIS) registered provider.
At Connect2Care, we can support clients with NDIS funding to increase their independence, improve their community access and, where appropriate, strive to participate economically in society. Our founders have been working with children and adults with disabilities for nearly two decades, and have a deep understanding of the complex nature of the NDIS and therapeutic disability support services. It is our experience and dedication that have made us a trusted name in the NDIS provider list, VIC.
We strive to make your journey through the NDIS a seamless one and have specialised NDIS consultants on hand to assist you with navigating
the process.
An NDIS provider is an individual or organisation that delivers support or product to an NDIS participant. An individual or an organisation can become an NDIS provider after registering with the NDIS Quality and Safeguards Commission (NDIS Commission).
You can find a list of registered NDIS providers in Victoria (VIC) by visiting the Provider Finder on the NDIS portal and checking the box corresponding to VIC under the State/Territory menu on the left-hand side of the page. You can also narrow your search by entering the Suburb name or postcode in the search box.
You can find registered NDIS providers near you by entering the name of your suburb or postcode in the search box on the NDIS portal’s Provider Finder page.
Registered providers are approved by the National Disability Insurance Agency (NDIA) to support plan-managed participants. Unregistered providers either lack approval or do not register with the NDIS. Unregistered providers aren’t accountable to the NDIA and can’t be investigated by them. They also can’t work directly with NDIA-managed participants, limiting their client base. Most unregistered providers are small private or family operators offering limited disability services.
If a service provider is registered with the NDIS, the provider’s name should be available on the NDIS portal’s Provider Finder database. You can enter the Australian Business Number (ABN) or the name of the provider in the search field on the left-hand side of the page.
NDIS support aims to provide people with disabilities equal life opportunities, including housing, employment, leisure activities, and social connections. Participants select and fund services from a personalised budget aligned with their goals.
The support framework consists of three main categories:

You can use unregistered and registered service providers if you have a plan-managed or self-managed plan. However, you can only use registered providers if your plan is NDIA-managed.
